There are several types of questions in this survey. Unless otherwise indicated, please click the response that best answers the question from your perspective. You will find two additional options, "Don't Know" (self-explanatory) and "N/A," meaning "not applicable to your particular involvement." Please answer the open-ended questions as completely as possible. Most questions must be answered before moving on to the next question. You may also go back at any time and change your answers.
